Prepare for Your Pet’s Boarding Stay

Before your pet’s boarding check-in, please fill out this form to give us permission to care for them and administer treatments if needed. It helps us provide safe, worry-free care during their stay.

Please Note: If your pet is currently on a prescription diet and you do not bring their food, additional charges will occur in the event that we have to supply a specialized diet.

Please note that a $6.00 fee will be charged for medication administration (per dosing time, not per medication).

A complimentary bath will be provided for all dogs staying with us for 5 nights or longer. A discounted nail trim of $15 is also offered to patients staying with us 5 nights or longer.

For the health of all our patients, boarding pets must have proof of current vaccinations and parasite screens administered by a licensed veterinarian. Your pet must also be free of external parasites. If fleas and ticks are noted upon arrival, your pet will be treated at your expense. We do not recommend leaving personal belongings with your pet. St. Francis will not be responsible for the return or condition of left belongings. (This includes blankets, toys, collars, harnesses and leashes).

Requirements for all boarding patients are as follows:
Canine :

Rabies vaccine within the past 12 or 36 months**

DHLPP vaccine within the past 12 months

Flu (H3N2/H3N8) vaccine within the past 12 months

Bordetella vaccine within the past 6 months

Intestinal Parasite Screening* within the past 6 months

Feline :

Rabies vaccine within the past 12 or 36 months**

FVRCP vaccine within the past 12 months

Leukemia vaccine within the past 12 months

Intestinal Parasite Screen* within the past 6 months

*A prescription broad-spectrum deworming can substitute for the intestinal parasite screen.

** In the event a 3 year vaccine was received.
